Click link above and provide a valid email address and mobile phone number. Choose the condition you are interested in from the drop-down list on the form. We will not share this information with anyone.

3 EASY STEPS TO GET READY FOR YOUR APPOINTMENT

1. Medical history and online sign-up

Prior to your appointment, we will send you an email invite to the patient information portal. This is where you fill out a secure medical questionnaire so that we can serve you better. You will be asked to sign information disclosure statements per HIPAA and Federal and State requirements.

2. Insurance verification

Prior to your appointment, check that the patient’s primary and secondary insurance plans cover specialty surgery services. If in doubt, call the insurance companies and ask what the out of pocket costs will be for the consultation or surgery.

We will confirm the insurance information you provide to us electronically prior to your visit, but we have no control over what the insurance will ultimately pay on your behalf. Financial responsibility for medical payments is with the patient. For specific questions especially if you are a private (self pay) patient, we will put you in touch with our billing company.

3. Imaging studies and test results

You will be asked to bring reports of test results, and the hard copies (CD/DVD) of CT and MRI studies to determine the need for surgery, and to allow for accurate surgery planning.

Some of the local imaging facilities provide online access, so you will not need to bring hard copies if your imaging was done at any of these centers:

  • VRI (Valley Radiology Imaging)
  • El Camino Hospital
  • Good Samaritan Hospital
  • Regional Medical Center of San Jose

OUT OF TOWN PATIENTS AND VIRTUAL APPOINTMENTS:

We may ask that you mail us your imaging studies prior to your appointment. Please mail studies at least 7 workdays prior to your appointment. Please use our MAILING ADDRESS. We will promptly return your imaging studies by USPS. We do not store any physical medical data or imaging studies.

Please do not send any medical information or imaging studies without explicitly being asked to do so. We do not accept any unsolicited mailings, and will either shred or return to sender.

PATIENT PORTAL SECURE COMMUNICATION, DO NOT USE E-MAIL FOR SENSITIVE INFORMATION

E-mail is not considered safe per HIPAA privacy laws because it may be breached and intercepted en route. We do not engage in email communication other than for brief messages that do not contain any Protected Health Information (PHI).

We communicate securely with our patients through our patient portal. The patient portal has double encryption and two-step sign-in verification. Please ask our staff at time of your visit to sign you up for patient portal access, should you be interested in accessing your medical information.
